The Vital Records Request Service allows users to request certified copies of the following vital record certificates: birth, death, marriage, civil union, divorce, or civil union dissolution. The service is provided by a third party in partnership with the State of Vermont and the Vermont Department of Health. Credit cards are processed at a higher total price through Vermont.gov, the state's official web portal, and supports the state's websites and online services.

What you should know about this Request Service:

• You may order as many copies or different certificates as you want on one purchase order.
• Each certified copy costs $12.00 through Vermont.gov.
• Most orders will ship via standard US mail within 5 to 7 business days of your request.
• For faster delivery, expedited shipping is available at an additional flat rate of $14.50. Expedited orders will be shipped overnight on the day your order is fulfilled.
• If your order can't be fulfilled, you will receive a partial refund (you pay only the search fee and the on-line service fee for any copy ordered but not shipped).
